What is CCR?
UC San Diego's Co-Curricular Record (CCR) highlights student involvement and achievements in opportunities beyond the classroom. The official CCR Transcript displays a student's verified positions including time period, position title, short description and associated Competencies (transferable skills). Position verification occurs through the Validation process.
What does this mean for you in PHUO?
Become a Health Activist!
Health Activist description:

High-achieving member that is actively involved in PHUO
-
Engage and contribute to the community on & off-campus
-
May increase chances during executive board applications in the Spring
Health Activist requirements:
-
30 hours / year
-
Attend at least 1 GBM per quarter

Attend at least 2 events per quarter
-
Complete 1-page reflection paper at the end of the year

DOES NOT require any formal application! Just show up to our events & make sure you’re recording it in the google form (linked below)
Tracker Form
CCR & General Member Tracker: after every event, you are responsible for
completing the CCR & General Member Tracker form

The form will be the same/continuous throughout the year (aka same link).

Each event will be worth hours, ex: GBM = 1 hour, and will vary.

PHOTO EVIDENCE IS REQUIRED (except GBMs): in order to prove that you
were at the event, fundraiser, etc. you must submit some sort of photo with
you in it!-
for GBMs: complete attendance form and mark that you want your
attendance to be counted towards CCR!

Record Keeping
Will I receive hours towards CCR if I attended your previous GBMs/events from this quarter?
YES! As long as you complete the attendance form at the end of this meeting and indicate you would like your attendance to be recorded for CCR, all your past participation (GBM #1, Porchlight Community Service) will be accounted for. You should have already completed attendance forms for these specific events in the
past and we will pull from them, so no need to complete anything further.
If you are ever curious as to how many hours you have completed just shoot us an email and we will check for you!
What if I don’t want to receive CCR credit and just want to be a general member?
That is totally OK too! Requirements to be a general member are less strict, just simply attend at least 1 GBM and 1 event per quarter! You will still need to complete the attendance form (same as CCR form) and are responsible for taking picture proof at the event. We will have a separate list of General Members who are not planning to receive CCR and General Members who are planning to receive CCR (considered Health Activists).
